Are you passionate about helping others regain their independence? As a Reablement Worker, you will support adults aged 18 and over in their homes after hospital discharge. Your role will focus on empowering individuals to regain confidence and learn new skills in personal care and daily living. This is a rewarding opportunity to make a positive impact every day! Hours vary from 20-36 hours a week.
As a Reablement Worker, your key responsibilities will include:

Visiting individuals in their homes to provide tailored support based on their personal goals.

Assisting with daily living tasks, including personal care, meal preparation, and safe medication administration.

Promoting dignity, respect, and independence in every interaction.

Collaborating with a multi-disciplinary team to ensure the best outcomes for service users.

Maintaining accurate records and updating support plans as needed.
To succeed in this role, you will need:

A Level 2 Diploma in Health and Social Care (or a willingness to complete this after induction).

A full driving licence and access to a vehicle for home visits.

Proficiency in using electronic devices for communication and record-keeping.

Knowledge of safe working practices and principles of dignity in care.
