Due to further investment and development we are seeking to appoint an enthusiastic, motivated and reliable Senior Community Care Support Worker, who is keen to deliver a range of quality care services to patients visited by the Community Response Hubs based across the Western Cheshire Footprint at Ellesmere Port, Chester and Tarporley.
As part of a team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars, rehabilitation and ideally have experience of providing care for palliative and end of life patients. This post requires travel across the Western Cheshire footprint and lone working, access to transport for work purposes is essential.
An enthusiasm to work as part of a team which delivers high quality services is a must for this role and you will also need to demonstrate excellent communication and interpersonal skills with the ability to be flexible, reliable and approachable.
The hours are full time 37.5 per week between the hours 08.00-22.00 including Bank holidays and weekends.
To assess, treat and discharge patients within agreed protocols and guidelines without direct supervision, escalating the need for support or guidance to qualified staff.
Within a defined caseload recognises precautions, signs/symptoms and
contraindications in relation to the delivery of care and seeks support of qualified staff.
Evaluate interventions within set protocols to meet the changing needs of patients as they progress with their therapy programmes.
Independently organise caseload and recognise clinical risk within own patient caseload escalating any concerns to qualified staff.
To be integral in discharge planning to ensure that all relevant and pertinent information in relation to discharge planning is collated on admission, potential issues that may affect discharge highlighted and documented.
To assess for and carry out criteria led discharge from therapy for patients identified as suitable by the qualified therapist.
Attend revelant MDT’s to clinical area.
Participate in the induction, training and education of students, nurses, nurse assistants and other junior staff as appropriate.
