Laindon Medical Group is a busy, patient-focused, and forward-thinking practice caring for over 18,000 patients in Laindon, Essex. We are rated Good by the CQC and work as part of West Basildon PCN alongside our neighbouring practices.
Alongside our GPs and nursing team we have an established clinical pharmacy team, with strong administrative and reception support.
The role
You will join our existing pharmacy team and take a key role in the safe and effective use of medicines across the practice in an on-site capacity. The post includes:
- Structured medication review, including polypharmacy and deprescribing
- Long-term condition review and optimisation
- Medicines titration and initiation within agreed protocols
- Medicines monitoring and shared care arrangements with secondary care
- Medicines safety, including MHRA alerts, clinical audits and incident review
- Clinical audit and quality improvement work
- Supporting the prescribing clerk and wider team with prescribing queries and repeat prescribing processes
- Contributing to prescribing budget and formulary work
About you
Essential
- Registered with the General Pharmaceutical Council (GPhC)
- Ability to work independently and as part of a multidisciplinary team
- Good communication skills and a patient-centred approach
Desirable
- Independent prescriber, or working towards
- Previous experience in general practice or another primary care setting
- Completion of (or enrolment on) the CPPE Primary Care Pharmacy Education Pathway
- Experience of SystmOne
- Member of the Royal College of Pharmacy
