Job Title: Support Worker

Job Purpose
The Support Worker will provide high-quality, person-centred support to individuals in a supported living environment, promoting their independence and well-being. This includes assisting with daily activities, personal care, household tasks, and social engagement while ensuring dignity, respect, and safety. The role requires collaboration with other professionals, families, and team members to create a supportive and tailored environment for each individual.
Key Responsibilities
- Provide direct care and emotional support to individuals, following their person-centred care plans.
- Assist with all aspects of personal care, including bathing, dressing, grooming, toileting, and mobility support.
- Support individuals with meal preparation, ensuring dietary requirements and preferences are met.
- Medication management and maintain accurate records in line with company policies and procedures.
- Encourage and promote independent living skills such as budgeting, shopping, and cooking.
- Facilitate access to community services, leisure activities, education, and employment opportunities.
- Monitor and report any changes in individuals' physical and emotional health to relevant professionals.
- Maintain accurate, up-to-date records and documentation in compliance with regulatory and company standards.
- Provide companionship and emotional reassurance, building trusting and professional relationships.
- Support individuals with behavioural challenges using agreed positive behaviour strategies.
- Work collaboratively with families, external agencies, and multidisciplinary teams to ensure a holistic approach.
- Adhere to safeguarding procedures and report concerns promptly.
- Comply with health & safety regulations, infection control procedures, and risk assessments.
- Participate in team meetings, training, and professional development activities.

- Previous experience in a care or support role.
- A compassionate, patient, and empathetic approach to care.
- Strong communication and interpersonal skills.
- Ability to work both independently and as part of a team.
- Proactive and solution-focused with strong initiative.
- Respect for confidentiality and professionalism.
- Willingness to work flexible shifts, including evenings, weekends, and bank holidays.
Desirable
- Level 2 or 3 in Health & Social Care.
- Awareness of safeguarding, mental health, learning disabilities, and autism (training provided).
