- To work closely with the Clinical Pharmacy Team in delivering excellent clinical pharmacy services to our inpatient wards.
- To lead in the delivery of specialist mental health clinical pharmacy services and to inpatient services, based Silverwood at St. Peter’s Hospital as part of a multi-disciplinary approach for addressing the pharmaceutical care needs of people who use our services
- Pharmaceutical care planning and clinical review of prescriptions including developing strategies for managing complex cases
- Involvement of people who use our services in decisions about their medicines to optimise treatment, support adherence and improve outcomes
- To provide expert pharmaceutical advice and respond to complex enquiries from consultants, medical staff, non-medical prescribers, nurses and other members of the multidisciplinary team (MDT) and prioritise tasks according to the level of urgency
- Working to develop bespoke local guidelines and protocols to support clinicians and nursing staff within the Trust
- Provide expert medicines management advice and support and ensuring appropriate governance arrangements are in place for the management of medicines, including the monitoring of prescribing and medicines use.
