We are seeking a compassionate, reliable, and motivated Outreach Support Worker to join our team. The successful candidate will provide person centred support to vulnerable adults within the community, helping them to live independently and achieve their personal goals.
Key responsibilities:
● Provide high quality, person-centred support in the community.
● Assist individuals to attend appointments, shopping trips, and social activities.
● Promote independence and support clients with daily living skills.
● Administer or prompt medication in accordance with care plans and company policies.
● Maintain accurate records, support plans, and incident reports.
● Work collaboratively with families, healthcare professionals, and other agencies.
● Safeguard vulnerable adults and report any concerns appropriately.
● Encourage positive wellbeing, confidence, and community engagement.
Requirements:
● Previous experience in health and social care is desirable.
● Excellent communication and interpersonal skills.
● A caring, empathetic, and professional approach.
● Ability to work independently and as part of a team.
● A full UK driving licence and access to a vehicle is desirable (if applicable).
● Enhanced DBS check (or willingness to obtain one).
Flexibility is essential, with a mix of day, night, and weekend shifts:
- Day Shift: 08:00 - 20:00
- Night Shift: 20:00 – 07:45
