Job Overview
The Project Manager will lead delivery of a new project designed to reduce caring-related health inequalities by strengthening Carers & Communities’ work with Primary Care Networks and wider health partners. The role will focus on earlier identification of unpaid carers experiencing ill health, improved understanding of the barriers they face, and the development of practical, evidence-based approaches that improve support for carers and influence wider system change.
1. Lead mobilisation, planning and delivery of the CAF-funded “Closing the Gap” project.
2. Develop effective working relationships with Primary Care Networks, GP practices and wider health and social care partners.
3. Identify and engage unpaid carers experiencing health inequalities, particularly those with long-term or emerging health conditions.
4. Gather and analyse quantitative and qualitative data, including carer stories, to inform service design, quality improvement and impact reporting.
5. Embed learning across Carers & Communities and share insights with partners to influence practice, service development and future funding opportunities.
Main Duties and Responsibilities
1. Project mobilisation and delivery
· Develop and maintain a clear project plan, delivery timetable, risk register and learning log.
· Set up practical systems and processes for project delivery, monitoring, quality assurance and reporting.
· Coordinate the planned programme of work across Primary Care Networks, including mobilisation, engagement, delivery and review phases.
· Ensure delivery remains aligned with the aims of reducing health inequalities, improving carer wellbeing and building sustainable support models.
2. Partnership working with Primary Care Networks and health partners
· Build constructive relationships with PCNs, GP practices and relevant health and social care colleagues through a variety of communication channels.
· Agree practical working arrangements with partners, including expectations, referral routes, communication arrangements and shared learning.
· Work with partners to identify carers living with long-term conditions, experiencing emerging ill health or at risk of crisis.
· Support partners to understand the impact of caring on health and wellbeing and the value of earlier carer identification and support.
3. Carer engagement, insight and support pathways
· Engage directly with unpaid carers to understand their experiences, barriers, health needs and support priorities.
· Use guided conversations and appropriate tools to capture carer voice, including stories, themes, outcomes and learning.
· Connect carers to appropriate Carers & Communities services, health-based support and wider community or partner provision.
· Pay particular attention to seldom-heard carers and carers facing barriers linked to geography, culture, digital exclusion, employment, isolation or caring intensity.
4. Data, evaluation and impact reporting
· Collect and maintain accurate project data, including demographic information, carer characteristics, caring role, health conditions, challenges, barriers and services accessed.
· Use qualitative and quantitative evidence to understand what changes as a result of project activity.
· Contribute to impact reporting, including use of our Social Value Engine where appropriate.
· Produce clear updates, learning summaries and reports for internal governance, funders and partners.
5. Learning, improvement and system influence
· Identify themes, gaps and opportunities from carer insight and partner feedback.
· Support continuous improvement by helping the organisation test, review and embed new approaches.
· Share learning internally so that project insight strengthens wider service delivery and organisational expertise.
· Contribute to the development of replicable models, future funding bids and system-level conversations about carers, health inequalities and prevention.
General responsibilities
· Increase awareness of the needs of unpaid carers and promote Carers & Communities services.
· Provide a positive experience for people accessing Carers & Communities services, including unpaid carers, partners and internal colleagues.

Person Specification
Experience
Experience of managing projects or programmes with multiple stakeholders, clear outputs and reporting requirements
Experience of working with health, social care, voluntary sector or community partners
Experience of engaging with people who may be under pressure, isolated, excluded or facing barriers to support
Experience of collecting, analysing and presenting data, feedback or impact evidence
Experience of working with Primary Care Networks, GP Practices or Integrated Care System partners
Knowledge
Understanding of the needs, rights and experiences of unpaid carers
Knowledge of health inequalities, prevention, early intervention and the links between caring and health/wellbeing
Awareness of safeguarding, confidentiality, equality, diversity, inclusion and data protection responsibilities
Understanding of Primary Care Networks, GP Practice structures or the wider Integrated Care System
Skills
Confident written communication skills, including the ability to produce concise reports, briefings and learning summaries
Ability to handle sensitive information appropriately and work in a person-centred, respectful and inclusive way
Ability to use Microsoft 365 and digital systems to manage information, reporting and communications
