Kindred Community are looking for a compassionate and experienced Registered Nurse Team Leader to provide clinical leadership within a small, bespoke community care package supporting a client in her own home in Beeston, Nottingham.
This is an opportunity to become an integral part of a dedicated care team, working closely with the Case Manager, family and wider multidisciplinary team to provide safe, consistent, person-centred care and rehabilitation.
The role would suit an experienced RGN who enjoys community-based nursing, understands the needs of people living with brain injury, and is confident providing clinical guidance and leadership while working collaboratively with other professionals.
About the role
This is a flexible part-time position of up to 15 hours per week. Candidates seeking fewer hours are also welcome to apply, and this can be considered depending on individual availability and suitability for the role.
You will provide clinical oversight to a dedicated 2:1 care team, helping ensure care is delivered safely, consistently and in line with the client's individual needs, rehabilitation goals, care plans and clinical recommendations.
Our client is a 67-year-old woman living in her own home with a brain injury and significant orthopaedic rehabilitation needs. You will work alongside support staff, the Case Manager, family, GP, therapists and other treating professionals to help promote dignity, independence, rehabilitation and quality of life.
Key responsibilities
- Provide clinical leadership and oversight of the client's health needs, care plans, protocols and risk assessments.
- Provide guidance, coaching and clinical support to the 2:1 care team.
- Monitor changes in health and presentation, ensuring appropriate action and escalation where required.
- Liaise effectively with the Case Manager, family, GP, therapists, care agency and wider MDT.
- Help ensure clinical and therapy recommendations are understood and consistently implemented.
- Support rehabilitation goals and promote a consistent approach across the care team.
- Undertake clinical tasks as required, including blood taking, PEG Care and medication management.
- Contribute to appointments, community activities and meaningful daily routines where appropriate.
About you
We are looking for a Registered Nurse with strong clinical judgement, excellent communication skills and a genuine commitment to person-centred care.
You will need:
- Current NMC registration and PIN.
- Proven experience working with people with acquired brain injury, neurological conditions, rehabilitation or complex care needs.
- Experience working collaboratively with GPs, therapists, consultants and other healthcare professionals.
- Confidence working autonomously and making sound clinical decisions.
- Competence and confidence in venepuncture/blood taking.
- Excellent spoken and written English, with the ability to communicate clearly and professionally with the client, family, care team and healthcare professionals.
- Strong leadership and interpersonal skills, with the ability to guide and support others.
- A calm, organised and professional approach.
- A full UK driving licence.
Knowledge of Nottingham, Beeston and local community services would be advantageous.

DETAILED JOB DESCRIPTION
Key Responsibilities
Clinical Oversight & Governance
· Provide ongoing professional oversight of the client’s health needs, rehabilitation goals, and overall wellbeing.
· Review and update care plans, clinical protocols, and risk assessments, ensuring they remain accurate, person-centred, and aligned with best practice.
· Monitor and escalate any changes in clinical presentation, ensuring timely communication with the treating team and GP.
Team Leadership & Coordination
· Offer clinical guidance and support to the 2:1 support team, promoting safe, consistent, and compassionate care.
· Liaise with the external care agency to ensure seamless communication, shared standards, and collaborative problem-solving.
· Contribute to staff development through reflective practice, feedback, and informal coaching.
Professional Liaison & Communication
· Act as a key point of contact for the multidisciplinary therapeutic team, including physiotherapy, occupational therapy, psychology, and other specialists.
· Attend or contribute to MDT meetings, providing updates and ensuring recommendations are implemented effectively.
· Maintain clear, professional documentation and communication pathways.
Administration & Coordination
· Manage the client’s clinical diary, including appointments, therapy sessions, and routine health monitoring.
· Conduct research into appropriate community activities, rehabilitation opportunities, and meaningful engagement options.
· Support with administrative tasks related to the smooth running of the care package.
Essential Criteria
· Registered Nurse (NMC) with current PIN.
· Experience in rehabilitation, neuro nursing, or complex care.
· Strong clinical judgement and confidence in autonomous decision-making.
· Driver – confident to drive the clients WAV to support community visits and appointments
· Excellent communication skills, able to work collaboratively with a professional treating team.
· Compassionate, supportive, and committed to promoting dignity, independence, and quality of life.
Desirable Criteria
· Experience in trauma nursing or orthopaedic rehabilitation.
· Previous experience supporting individuals with acquired brain injury in the community.
· Understanding of person-centred, goal-focused rehabilitation approaches.
· Full driving licence

KEY TASKS
1. To follow the agreed support and rehabilitation approaches, being aware of the clients cognitive and physical limitations.
2. To work with the client in providing and following plans and structures for the week.
3. To safeguard and promote at all times the client’s physical and mental wellbeing comfort and safety.
4. To communicate with family members and relevant professionals as necessary.
5. To work in collaboration with other senior staff members, deputy team leaders, team leaders and Case Managers to ensure the smooth running of the team and services delivered to the client.
MAIN DUTIES AND RESPONSIBILITIES
1.1 Through initial training and ongoing development maintain knowledge and understanding of the problems and repercussions of the clients traumatic brain injury.
1.2 Maintain support and communication routines established by professionals and family involved ensuring maintenance and further development of the clients abilities, goals and rehabilitation.
1.3 Participate in the maintenance of Neuropsychology, Physiotherapy, Occupational, Speech & Language and other specified programmes prescribed by the treating specialists.
1.4 Undertake those aspects of personal care that the client is unable to do for herself alongside the agency support staff and assist in those other aspects where requested to do so by her.
1.5 Maintained accurate daily records of support and rehabilitation undertaken and other records required from time to time.
2.1 Adopt a joint working approach to your team when setting up and maintaining a stimulating programme of activities with the client.
2.2 Work collaboratively and share information with other team members in ensuring the client’s rehabilitation goals are achieved.
2.3 Provide a mentoring role to your colleagues by sharing knowledge/skills to facilitate development of staff ability and understanding within their role.
3.1 Undertake and contribute to Risk Assessments as necessary in order to ensure the health and safety of both the client and other team members.
3.2 Ensure the client is not placed in situations, which will put them at risk. Should this occur, follow the agreed plan or if unidentified risk, take reasonable action to deal with the situation.
4.1 Respect the need for confidentiality when possessing knowledge of a private and personal nature about the client and family.
4.2 Endeavour not to become over involved in the family affairs in a personal fashion.
4.3 Communicate to the Team Leader/Case Manager any work with the client and deviation from normal functioning via the agreed communication system.
Support agency staff and management in the arrangement of training and monitoring of training provision for the support team. Ensuring that all staff have up to date training and that if there are gaps in training provision that these are filled in a timely manner.
Participate in training and development provided in order to increase knowledge base from time to time.
5.2 Ensure that records of hours worked are maintained and returned to the agreed timetable.
