- Work collaboratively with professionals including nurses, physiotherapists, occupational therapists, social workers, and voluntary sector partners
You will be part of an established reablement and locality team, working across agencies including Community Equipment Services, Telecare, and voluntary organisations. The service focuses on enabling individuals to regain and maintain independence while strengthening community connections.
