Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will be responsible for organising and overseeing care plans within a Domiciliary care environment, ensuring the highest standards of senior care are maintained. The Care Coordinator will work closely with care staff, The position is paid and requires a proactive individual with excellent organisational and communication skills.
Responsibilities
- Develop, implement, and review personalised care plans tailored to each resident’s needs.
- Supervise and support care staff to ensure compliance with care standards and procedures.
- Coordinate daily activities and schedules to optimise resident wellbeing and comfort.
- Liaise with healthcare professionals, families, and external agencies to facilitate seamless care delivery.
- Maintain accurate documentation of care activities, incidents, and updates in IT systems.
- Organise training sessions for staff to ensure ongoing professional development and adherence to best practices.
- Ensure compliance with health and safety regulations within the care environment.
Skills
- Proven experience in supervising or managing staff within a Domiciliary care or senior care setting.
- Strong leadership qualities with the ability to motivate and guide a team effectively.
- Excellent communication skills, capable of liaising confidently with residents, families, healthcare professionals, and team members.
- Valid driving licence is preferred for travel between locations or appointments.
- Proficiency in IT systems for maintaining accurate records and updating care plans efficiently.
- Knowledge of care planning processes, health & safety regulations, and safeguarding procedures.
- Organised with strong attention to detail to ensure high standards of resident care are consistently met.
