This role involves supporting individuals with learning disabilities and complex needs within a registered care home setting. The post holder will promote health, independence, and wellbeing by providing high-quality, person-centred care in line with care plans developed by a multidisciplinary team. Working within a Positive Behaviour Support framework, they will deliver accessible, non-stigmatising support tailored to individual needs. The role includes implementing care plans, monitoring progress, and providing regular feedback to professionals to support ongoing evaluation and improvement of care.
Based within Learning Disabilities Care Home Services, the post holder will work flexibly across social care and health teams as required, contributing to the delivery of specialist, localised services that enhance quality of life for individuals with learning disabilities.
We are a dedicated residential social care home providing support to six ladies aged 40–75. We support people with severe learning disabilities, autism, and other complex needs. Our ladies lead very active social lives and enjoy engaging in many leisure pursuits outside of the home, visiting family and friends, going on outings and holidays, having new experiences.
Support individuals with daily living, promoting independence, working alongside and adhering to the persons support plan and risk assessments.
Supporting with personal care.
Supporting with health appointments
Supporting people to go out to places of interest, go on holiday.
Supporting making and cooking meals and drinks.
Shift leading, ensuring a shift runs safely and smoothly.
Administration of medication.
Escalating any concerns around the health of an individual.
