Responsibilities
- Deliver personalized care plans that meet each client’s unique needs, promoting comfort and safety
- Assist clients with daily activities such as mobility, personal hygiene, medication management, and nutrition
- Support clients in maintaining their independence by encouraging participation in activities and routines
- Communicate effectively with clients, families, healthcare professionals, and team members to coordinate care delivery
- Drive to different locations as needed for home care visits or appointments, ensuring punctuality and reliability
- Maintain accurate documentation of care provided and update client records using IT systems
- Ensure compliance with health and safety standards and report any concerns or incidents promptly
Qualifications
- Experience working in a care home or providing home care services is highly desirable
- Ability to develop and follow detailed care plans tailored to individual needs
- Valid driving license and ability to travel between locations confidently
- Strong communication skills in English, both verbal and written
- Proficiency with IT systems for record keeping and communication purposes
- Excellent interpersonal skills, demonstrating empathy, patience, and professionalism
- Knowledge of health and safety regulations relevant to care environments
