Care Bridge Specialists are looking for experienced Support Practitioners to join a dedicated team providing long-term wraparound support to a young person with complex needs in the Thames Valley area.
This is an ongoing package of care with 12-hour day and night shifts (09:00–21:00 / 21:00–09:00). We are building a consistent team around the young person, so this will be an ongoing rolling rota of the same weekly shifts as reliability, continuity and the ability to build a trusting relationship are really important.
The Role
You will be supporting a young person with autism, profound and multiple learning disabilities (PMLD), significant communication and sensory needs and frequent, intense behaviours of concern.
Support will include:
- Following an established daily routine, sensory diet and weekly activity schedule
- Supporting appointments, tutoring and meaningful activities outside the home
- Positive Behaviour Support (PBS) and proactive approaches to behaviours of concern
- Supporting communication, choice and independence
- Working closely and consistently with family and the wider professional team
- Accurate recording and following individual care plans and risk assessments
What We Are Looking For
You will need:
§ 3 years+ experience supporting children or young people with complex needs. NVQ Level 2/ 3 is desirable or Care Certificate minimum.
§ Strong experience of autism and profound and multiple learning disabilities (PMLD)
§ Confidence supporting frequent and intense behaviours of concern
§ A good understanding of Positive Behaviour Support (PBS), sensory needs and de-escalation, supported by appropriate training/certification such as NAPPI, iPBS, PROACT-SCIPr-UK® or Team Teach
§ Experience of supporting individuals where restrictive physical interventions may form part of an agreed behaviour support and risk management plan, including low-level holds, with a clear understanding that physical intervention is used only when necessary and proportionate. PMVA/CPI Safety Intervention or equivalent accredited training would be advantageous
§ Confidence undertaking dynamic risk assessments and responding appropriately to changes in presentation, environment and risk
§ Ability to produce clear, accurate and professional care notes, incident records and other documentation
§ Confidence working within a young person’s family home, building a positive and respectful relationship with parents/family and maintaining clear professional communication and boundaries.
§ Excellent reliability and a commitment to working as part of a consistent, long-term team
§ Full UK driving licence and own vehicle with business insurance – essential, as driving is an integral part of the role
Availability to commence the package from 24 September 2026
