With YOU in Mind Community Care is a growing, nurse-led home care provider supporting people to remain safely and independently in their own homes across Barnsley and surrounding areas.
We are recruiting experienced Community Care Assistants to join our team in permanent, guaranteed full-time positions.

Our service operates between 7am and 11pm, 7 days a week, therefore flexibility to work a mixture of shifts, including weekends and bank holidays, is essential.

Skills
Previous paid community/domiciliary care experience is essential.
Successful applicants must:
- Have experience providing care and support to people within their own homes
- Have a full UK driving licence and access to their own vehicle
- Have a good standard of spoken, reading and written English
- Communicate confidently and professionally with clients, relatives, colleagues and healthcare professionals
- Be able to read, understand and accurately complete care plans, risk assessments, medication records and care documentation
- Understand person-centred care, dignity, choice and independence
- Be confident supporting people with a range of care needs
- Have good observational skills and recognise and report changes in a client’s presentation or wellbeing
- Be reliable, punctual and able to work independently in the community
- Work effectively as part of a wider care team
- Maintain professional boundaries and confidentiality
- Be flexible and dependable

Duties
As a Community Care Assistant, you will support clients within their own homes according to their individual care plans and preferences.
Your duties may include:
- Supporting with personal care, washing, dressing and continence needs
- Supporting clients to maintain their dignity, independence and choice
- Medication prompting, assistance or administration in accordance with care plans, training and company policy
- Supporting with mobility and safe moving and handling
- Preparing meals and drinks and supporting appropriate nutrition and hydration
- Providing companionship and emotional support
- Supporting clients with day-to-day activities within their own homes
- Observing clients’ physical and emotional wellbeing and promptly reporting concerns or changes
- Liaising appropriately with families, colleagues and healthcare professionals
- Accurately completing electronic care records and medication documentation
- Following individual care plans, risk assessments, safeguarding procedures and company policies
- Maintaining infection prevention and control standards
- Travelling between clients across Barnsley and surrounding areas
- Providing consistent, reliable and compassionate care at every visit
