Job Summary
To provide high quality, person centred support to adults with learning disabilities living in the community, promoting safety, wellbeing, independence, and inclusion. The Senior Care Worker leads by example, supports colleagues, and acts as the first point of contact for operational issues, safeguarding concerns, and day to day coordination. This is a full time role, working every other weekend, with on call cover included as part of the rota.
Key Responsibilities
1. Person Centred Support
- Deliver personalised care in line with care plans and risk assessments.
- Promote independence, dignity, choice, and community participation.
- Support with daily living tasks, medication, personal care, communication needs, and behaviour support strategies.
2. Leadership & Team Support
- Provide daily guidance to Care Workers and model professional standards.
- Support rota planning, shift coordination, and task allocation.
- Mentor new staff, support induction, and contribute to training and competency checks.
- Escalate performance or conduct concerns to the Team leader and Registered Manager.
3. Safeguarding & Risk Management
- Identify and report safeguarding concerns promptly.
- Complete and review risk assessments, incident reports, and behaviour support plans.
- Ensure safe lone working practices and adherence to community safety protocols.
- Support individuals in line with the Mental Capacity Act and best interest processes.
4. Medication & Health Support
- Administer medication safely and complete MAR documentation accurately.
- Monitor health changes and liaise with healthcare professionals.
- Support individuals to attend appointments and engage with health services.
5. Documentation & Compliance
- Maintain accurate daily notes, incident forms, safeguarding logs, and care plan updates.
- Ensure compliance with CQC regulations, organisational policies, and LD best practice.
- Contribute to audits, quality checks, and service improvement.
6. Communication & Partnership Working
- Build positive relationships with families, advocates, social workers, housing providers, and community organisations.
- Support individuals to access social, educational, volunteering, and community activities.
- Communicate effectively using appropriate methods (e.g., Makaton, visual aids, easy read formats).
Essential Skills & Experience
- Experience supporting adults with learning disabilities.
- Strong understanding of PBS, safeguarding, MCA, DoLS, and CQC requirements.
- Confident leading shifts and supporting staff.
- Excellent communication skills and ability to adapt to different communication needs.
- Competent record keeping and digital systems.
Desirable
- NVQ/QCF Level 3 in Health & Social Care (or working towards).
- Experience with autism, complex needs, or behaviours of concern.
- Training in Makaton, TEACCH, or LD communication tools.
- Full UK driving licence.

Work Location: On the road
