Main Responsibilities:
- Oversee the development and implementation of tailored care plans for clients, ensuring they meet individual needs and preferences.
- Supervise and support care staff, providing guidance, mentorship, and leadership to maintain high standards of service delivery.
- Coordinate client appointments, liaising with healthcare professionals, family members, and other stakeholders to ensure seamless care provision.
- Manage daily schedules for care teams, optimizing routes and assignments to maximize efficiency and client satisfaction.
- Maintain accurate records using IT systems for care documentation, progress notes, and compliance reporting.
- Conduct regular visits to clients’ homes or care facilities to assess quality of care and address any issues promptly.
- Promote effective communication within the team and with clients, fostering a positive environment built on trust and transparency.
Breakdown of duties:
- Produce and maintain weekly care rotas.
- Allocate care workers according to skills, availability and service user needs.
- Manage last-minute changes and emergency cover.
- Minimise missed or late visits.
- Maintain accurate care records.
- Assist with care assessments and reviews.
- Liaise with service users and families.
- Respond to enquiries and concerns.
- Support with developing, reviewing, and updating Service User Care Plans and Risk Assessments.
- Maintain staff availability.
- Communicate schedules effectively.
- Monitor attendance and punctuality.
- Participate in the on-call rota.
- Conduct spot checks.
- Conduct Competency Assessments.
- Report safeguarding concerns.
- Escalate incidents appropriately.
- Ensure documentation is completed correctly.
- Maintain electronic care management systems.
- Prepare reports for management.
- Support recruitment and induction.
- Process timesheets and payroll information.
Work under the direction of the Registered Manager to support the daily running of the service.
Experience:
- Proven experience in senior or community care settings, with a strong understanding of care planning and delivery.
- Supervising or leadership experience within a care home or similar environment is highly desirable.
- Valid driving license with the ability to travel between different community locations as needed.
- Familiarity with creating and managing care plans using IT systems or electronic health records.
- Excellent communication skills to liaise effectively with clients, families, healthcare professionals, and team members.
- Demonstrated ability to supervise staff, provide leadership, and promote teamwork in a fast-paced environment.
Desirable:
- Experience using care management software (e.g., Access Care Planning, Birdie, Nourish or CareLineLive).
- NVQ Level 3 in Health & Social Care.
Pay: £27,500.00 per year
