Crossroads Gloucestershire is growing! Following the continued expansion of our organisation and the services we provide across the Forest of Dean and Gloucester, we are delighted to be creating a brand-new Operations & Quality Co-Ordinator position within our team.
This is an exciting opportunity for an experienced care professional to join our established and supportive organisation at an important time of growth. The successful candidate will play a key role in supporting our expanding workforce, maintaining high standards of care and ensuring our service continues to provide safe, effective and person-centred support.
Crossroads Gloucestershire is a specialist charitable organisation that has grown over the last twenty-five years to provide high-quality support services to enable people who are living with frailty, ill-health or disability, and their Carers, to lead safer and healthier lives.
The Role
The Operations & Quality Co-Ordinator will play a key role in ensuring we continue to provide safe, high-quality and person-centred care as Crossroads Gloucestershire expands.
A key focus of the role will be assessing new clients, developing person-centred care plans and risk assessments, and undertaking regular reviews of existing clients to ensure their care continues to meet their changing needs. You will work closely with clients, families and other professionals including Social Workers, GPs, hospitals, Occupational Therapists and District Nurses.
You will monitor the quality and effectiveness of care, identify changes in clients' needs and ensure appropriate action is taken. You will also provide guidance and support to care staff, undertaking observations, spot checks and probationary reviews where required, and helping staff to deliver care safely and in accordance with individual care plans and Crossroads Gloucestershire's policies and procedures.

Your responsibilities will include:
· Carry out initial assessments of new clients, identifying their care and support needs and how Crossroads Gloucestershire can best meet them.
· Develop and regularly review person-centred care plans and risk assessments, including six-monthly reviews and reviews following changes in clients' needs.
· Monitor the quality and effectiveness of care, ensuring clients' needs, preferences and agreed outcomes continue to be met.
· Build positive relationships with clients, families and advocates, and liaise with Social Workers, GPs, hospitals, Occupational Therapists, District Nurses and other professionals as required.
· Identify changes in clients' needs and ensure appropriate action, escalation or professional referrals are made.
· Support care staff through guidance, observations, spot checks and probationary reviews, ensuring they have the knowledge and information needed to deliver safe, person-centred care. Operations: Work closely with management and rostering teams to ensure care packages are delivered safely and effectively, while contributing to quality assurance, service improvement and the continued growth of Crossroads Gloucestershire.
Working Pattern
This is a full-time position of 37.5 hours per week, working on a rotating pattern:
Week without on-call: Monday to Friday
Week with on-call: Monday, Tuesday, Friday, Saturday and Sunday
There is also a requirement to hold the On-Call telephone during the weekend worked. An additional enhancement is paid for this responsibility outside of your normal working hours.
About You
We are looking for an experienced and enthusiastic care professional who is passionate about providing excellent standards of care and supporting others to achieve their potential.
You will hold a Level 3 qualification in Health & Social Care and ideally have experience within a care or supervisory environment.
You will need to demonstrate:

- Excellent communication and interpersonal skills.
- An approachable, supportive and professional manner.
- The confidence to provide advice, guidance and constructive feedback to care staff.
- Good organisational and time-management skills.
- The ability to work independently whilst also being an effective member of a team.

- The ability to build positive relationships with clients, families, professionals and colleagues.
You will also need to hold a full driving licence, have access to your own vehicle and be willing to travel to Crossroads Gloucestershire service users' locations,
