WMHC Ltd is a growing and ambitious care provider delivering high-quality home care and supported living services. We are seeking a driven and experienced Clinical Referrals Manager to join our team and lead the end-to-end process of securing and mobilising new care packages.

About the Role
As a Clinical Referrals Manager, you will oversee the full referral journey — from initial enquiry through to assessment, mobilisation, and service commencement. You will act as the key link between commissioning bodies (NHS, Local Authorities, ICBs) and our operational teams, ensuring care packages are safe, compliant, and person-centred.
This role combines clinical understanding, relationship management, and business development to drive sustainable growth.
Key Responsibilities
- Receive, review, and process referrals from Local Authorities, NHS Continuing Healthcare (CHC) teams, case managers, and commissioners
- Conduct detailed assessments of client needs, including complex care, autism, dementia, and mental health
- Match service users with appropriate staffing and resources
- Build and maintain strong relationships with NHS Integrated Care Boards (ICBs), commissioners, social workers, and families
- Lead the mobilisation of new care packages, working closely with clinical and HR teams
- Ensure all referrals and care plans meet Care Quality Commission (CQC) standards and internal compliance policies
- Track referral pipelines, monitor conversion rates, and identify market growth opportunities
- Contribute to business growth by consistently meeting and exceeding referral targets
Requirements
Experience:
- Previous experience in business development, referrals management, care management, or a similar role within health or social care
Knowledge:
- Strong understanding of the UK care sector
- Experience with complex care, autism, dementia, and mental health services
- Knowledge of commissioning pathways and NHS/Local Authority referral processes
Qualifications:
- NVQ Level 4 or 5 in Health & Social Care or
- Clinical qualification with active NMC PIN (for complex care background)
Skills:
- Excellent communication and negotiation skills
- Strong IT skills (Microsoft Office and care management systems)
- Ability to manage multiple referrals efficiently
- Commercially aware and target-driven
Additional Requirements:
- Full UK driving licence
- Clear enhanced DBS
- Long-term commitment and ambition to grow within the role

- Basic salary of £40,000 per annum
- Uncapped commission structure with realistic OTE of £80,000 per annum
