About Mentis Midlands
Mentis Midlands is a growing mental health charity providing supported accommodation and person-centred support to adults experiencing mental health challenges across the West Midlands. Our mission is to empower individuals to achieve greater independence, improve their wellbeing, and successfully move on to independent living.
We are looking for a compassionate, organised and motivated Support Worker to join our expanding team.
The Role
This is a hybrid role, with the majority of your working day spent visiting residents in the community. When you are not carrying out support sessions or attending meetings, you will be expected to work from home to complete administration, case notes, support plans and other associated tasks.
You will manage a caseload of approximately 30 residents, providing weekly support sessions, welfare checks and person-centred support to help individuals maintain their tenancy, improve their mental wellbeing and work towards independent living.
Travel is an essential part of this role and may include properties located across Birmingham, Sandwell, Dudley, Coventry, Nuneaton and Tamworth. We understand the importance of maintaining a healthy work-life balance, so your schedule will be carefully planned to provide the most efficient travel routes wherever possible, ensuring both employee wellbeing and operational efficiency.
Key Responsibilities
- Manage a caseload of approximately 30 residents.
- Deliver weekly one-to-one support sessions and welfare checks.
- Complete and regularly review support plans and risk assessments.
- Maintain accurate, detailed and timely case notes using Microsoft Office and internal systems.
- Support residents with budgeting, benefits, tenancy sustainment and independent living skills.
- Encourage engagement with healthcare professionals, local authorities and community services.
- Promote positive mental health and recovery-focused support.
- Liaise professionally with external agencies and partner organisations.
- Respond appropriately to safeguarding concerns and follow organisational policies.
- Work independently while effectively managing your own diary and workload.
About You
We are looking for someone who is:

- Well organised with excellent time management skills.
- Comfortable working in a hybrid environment.
- Able to build positive, professional relationships with residents and partner agencies.
- Confident using technology and able to work efficiently with Microsoft Office, including Word, Excel, Outlook and Teams.
Essential Requirements
- Full UK driving licence.
- Access to a reliable vehicle insured for business use.
- Willingness to travel regularly throughout the West Midlands.
- Good working knowledge of Microsoft Office.
- Excellent written and verbal communication skills.
- Experience within mental health, supported housing, social care or a related support role.
- Enhanced DBS check (or willingness to obtain one).
- Right to work in the UK.
Desirable
- Level 2 or Level 3 qualification in Health & Social Care (or equivalent).
- Knowledge of safeguarding procedures and mental health recovery.
- Experience working with vulnerable adults within community settings.
