About the Role
As a Care Coordinator, you’ll be at the heart of our care team, helping to ensure clients receive safe, consistent and personalised care while supporting our carers and clinical team.
You will:

Coordinate and maintain care rotas up to six weeks in advance.

Build positive, trusting relationships with clients and carers.

Match carers to clients based on availability, preferences and skills.

Work closely with the clinical team and keep them updated on client needs.

Manage the on-call rota and provide out-of-hours support.

Conduct client home visits when required.

Support the recruitment and welcoming of new staff.

Answer calls and provide professional, friendly support.

Use digital care systems such as Birdie and Microsoft Office.

Contribute to a positive, supportive and welcoming office environment.

Take initiative, stay organised and ensure day-to-day operations run smoothly.

Full UK driving licence and access to own transport required.
