Care Coordinator role at Alicelyn Healthcare
£29,120.00 per year (plus on-call payment of £130 per weekend on a 1 week in 4 weeks rota).
Alicelyn Healthcare is a Domiciliary Care Company supporting Service Users within Rotherham and Sheffield. We are expanding and working with both Rotherham and Sheffield Councils on the supported living frameworks, and are now looking for a Care Coordinator to join our team.
We are looking for someone who has experience within Domiciliary Care and ideally experience using Care Planner software, with Care qualifications. However, we are willing to support the right candidates through their training up to level 3.
We are looking for someone who can work as part of a team as well as confident to work on their initiative. A good communicator, respectful, considerate, empathetic, time conscious, and considered computer literate (knowledge in Excel, Word and other applications), and willing to learn continuously.
Report to: Deputy Care Service Manager/ Care Service Manager
Purpose of the role
- To ensure the Service Users receive excellent quality care and support to keep them safe and comfortable in their own homes, as well as promoting independence, with a consistent continuity of carer and cares, whilst adhering to the Care and Support Plan. Striving to achieve optimised scheduling that meets the needs of both Service Users and Carers.
Job Overview
- Liaise with Management to ensure the right number of care staff with the right skills are recruited to meet the needs of the Business.
- Arrange cover for care staff, Sickness and annual leave and absenteeism. Report any issues with Compliance to Management.
Location
- Working at Alicelyn Healthcare office in Eastwood, Rotherham.
- Working hours are 8 hours a day, Monday to Friday.
Role-specific Duties
- Accept, Allocate and process new Service Users' referrals for care and support promptly and effectively.
- Process changes to Service Users' required care needs and time changes.
- Ensure there is sufficient cover to deliver care and support to all Service Users on time and in alignment with their personal care plans.
- Work with carers to ensure they are maintaining an appropriate work/life balance.
- Schedule Care staff to provide care and support to Service Users, working with other members of the Alicelyn Healthcare workforce to appropriately match Care staff to Service Users taking into account, Care staff skills, Experience and availability to safely deliver the care and support plans, Also taking into account the Service Users preferences and care needs, whilst thinking about travel arrangements, routes and working patterns to make efficient use of Care staff time whilst meeting the needs of the Service Users.
- Make Required adjustments to rotas as required to provide cover for Emergency situations, to cover staff holidays or Sickness, etc.
- Distribute the Carers rotas at agreed interviews, maintaining a 2-week rolling rota.
- If required take call in the out of hours emergency on-call rota, this will only be required after the post holder has received full training and has been assessed as competent to provide appropriate guidance and advice in Emergency situations.
- Work closely with other office staff to ensure electronic and handwritten records are all up to date, Using Careplanner for the rostering and monitoring Mileage and travel distances and times, also checking for late calls, hospital admissions, near misses, medication errors, falls etc, ensuring all Accident and Incidents are recorded, reported and acted upon and passed on to management.
- Keep all information regarding Service Users, their families and Staff secure and confidential.
- Work with other Alicelyn Healthcare staff and other Health and Social care Professionals to deliver high quality homecare services and to make improvements where necessary.
- Deal with complaints and compliments.
- Ensure that the services within Alicelyn Healthcare are compliant in accordance with legislation requirements, and regulations, in line with accepted best practice and within CQC regulations and adhered to by all staff.
