Location: Chesham
Hours: 40 hours per week, 5-day working pattern
As a Care Coordinator, you will play a vital role in ensuring the smooth and efficient delivery of care services. You will be responsible for coordinating care schedules, supporting recruitment activities, maintaining accurate records, and ensuring all client visits are covered by suitably matched Care Workers.

- Answer and manage incoming telephone enquiries professionally and efficiently.
- Coordinate and allocate Care & Support Workers to client visits, ensuring all calls are covered.
- Process new client referrals and prepare documentation for care assessments.
- Maintain accurate client and staff records, both electronically and in paper files.
- Ensure all office systems, procedures, and compliance requirements are followed.
- Work closely with the Field Care Supervisor to ensure high standards of service delivery.
- Update and maintain the care management system and office databases.
- Produce reports and management information as required.
- Ensure all documentation is accurate, compliant, and up to date.
- Be part of the out-of-hours on-call rota on a shared basis, responding to emergencies and supporting the delivery of care services outside normal office hours.
The ideal candidate will have:
- Previous experience in a care coordination, scheduling, or administrative role would be advantageous but is not essential, as full training will be provided.
- Excellent organisational and time-management skills.
- Strong communication and customer service abilities.
- Confidence using computer systems and Microsoft Office applications.
- The ability to work effectively in a fast-paced environment and manage multiple priorities.
- A positive, solution-focused attitude and strong attention to detail.
- Experience within domiciliary care or the health and social care sector is desirable but not essential.
