An exciting opportunity has become available to work as an experienced Social Worker within the Adult Contact Team South in the Adult Social Care department. The position is for 1 FTE (37 hours) per week over a Monday to Friday working pattern. The position is based at Delamere House in Crewe and covers the whole of the South locality.
As an experienced Social Worker, you will have the core skills and values of Social Work. Your practice will be underpinned by strength and asset-based approaches to support the residents of Cheshire East.
You will have the ability to work in partnership with other professionals, and alongside individuals, families and carers who contact the service; to achieve individual outcomes. You will support and take a preventative approach to reduce or delay individuals’ needs, and to enable independence.
With effective communication skills you will provide information and advice, and signpost people to community resources. You will also have a sound understanding and application of legislation to complete safeguarding enquiries, adult needs assessments, carers assessments, and person-centred support planning.
As an enthusiastic and empathic person you will have the opportunity to work creatively, flexibly and autonomously.
As an organisation we will support you with an induction, training and continuous professional development. You will be required to have an enhanced DBS, and a driving licence is essential.

The successful candidate will have:
- Extensive knowledge and experience of Social Work practice;
