We are seeking an enthusiastic Therapy Assistant Practitioner (TAP) to join our dynamic Community Rehabilitation and Short Term Assessment Team (CR-STAT) to work with Community Care Plus (CCP) Therapy Virtual ward pathway , in Greenwich.
As part of the Multi-Disciplinary Team (MDT), you will work under the guidance of allied health professional (AHP) colleagues to deliver rehabilitation care plans in patients’ own homes. You will help patients regain independence and maximise their rehabilitation potentials by implementing therapy programmes, prescribing appropriate equipment, and monitoring progress to ensure care is personalised and effective.
The CCP pathway supports Greenwich residents with early discharge from acute hospitals by providing timely, intensive rehabilitation in the community.
