We are excited to announce new opportunities within our Hampshire and Isle of Wight All Age Continuing Care service. As a Case Practitioner in Continuing Healthcare, you will be responsible for supporting the wider clinical team to conduct comprehensive individual assessments of patients (including completing CHC checklists) to assess their eligibility for NHS Continuing Healthcare (CHC) to enable timely provision of care, case managing clients under supervision to ensure that their needs are effectively met and promoting Personal Health Budgets and personalisation to empower people with complex health needs live their best lives.
Experience in Continuing Healthcare is helpful, but if you have the right skills and attitude, we can support you to develop the specific skills required. We offer team training opportunities, competency frameworks and Team Leaders who are non-case holding in order to mentor and develop our Case Practitioners, including safeguarding, Mental Capacity Act and Court of Protection. You will be part of a wider All Age team who provide a wide range of expertise and support.

Main duties, tasks & skills required
Provision of non-clinical support to the assessment, review and case management processes for NHS All Age Continuing Care
Responsible for supporting Clinical Assessor/Practitioners in their roles
Completion of CHC checklists
Preparation of Funding Request Forms under delegated authority
Completion of non-clinical case management tasks on behalf of Clinical Assessor/Practitioners; for example (but not limited to) liaising with patients/families, conducting reconciliation of 1:1 provision, liaising with care agencies around staffing and shift fill, supporting completion of care and support plans and Personal Health Budgets, monitoring Extra Contractual Referrals, conducting customer service/package of care follow-up enquiries
Communication with clients, families, carers, external agencies, NHS staff and other healthcare professionals in a sensitive manner where the nature of the communications may be emotive or complex
Act as a primary point of contact for all case management enquiries regarding the NHS All Age Continuing Care
Responsible for updating the care management systems with appropriate documentation
Supporting the team with the management of projects, gathering information and undertaking enquires
To gather evidence for appeals/disputes/Independent Review Panels/Parliamentary and Health Service Ombudsman, ensuring everything required is present

The All-Age Continuing Care Team supports over 7,500 clients through a range of funding arrangements including Continuing Healthcare, Section 117 aftercare, specialist placements, Children's Continuing Care and other joint funding and oversight of a budget of 392M. Our roles are community based with flexible working from a local office and home and is a key role to support people to live well with complex health and primary health needs. Post holders will be employed by Hampshire and Isle of Wight Healthcare NHS Foundation Trust, working in partnership with NHS Hampshire and Isle of Wight Integrated Care Board.
