Care XY Ltd is a growing domiciliary care provider delivering high-quality, person-centred care to individuals in their own homes. We are seeking an experienced and dedicated Care Coordinator to support the delivery of safe, effective, and compliant care services.
This role is primarily focused on completing care plans, conducting risk assessments, reviewing care needs and support the office team with day-to-day operational tasks, helping to ensure the smooth and efficient running of the service while maintaining excellent standards of service user care. The successful candidate will participate in a monthly on-call rota to support the delivery of a responsive and high-quality care service. You will work closely with service users, families, care staff, and healthcare professionals to ensure care is tailored to each individual's needs and regularly reviewed.
Key Responsibilities
- Complete initial service user assessments in their homes.
- Develop, review, and update person-centred care plans.
- Conduct and review risk assessments to ensure safe care delivery.
- Carry out service reviews and quality assurance visits.
- Monitor client outcomes and identify changing care needs.
- Liaise with clients, relatives, social workers, GPs, and healthcare professionals.
- Ensure all care documentation is accurate, up to date, and compliant with CQC standards.
- Support care staff with guidance regarding care plans and risk management.
- Participate in investigations, audits, and continuous improvement activities.
- Assist with care package set-ups and client onboarding.
- Provide office support and assist with rota management when required.
Essential Requirements
- Previous experience in domiciliary care as a Care Coordinator, Field Care Supervisor, Senior Carer, or similar role.
- Strong knowledge of care planning and risk assessment processes.
- Understanding of CQC regulations and person-centred care.
- Excellent written and verbal communication skills.
- Good IT skills and experience maintaining digital care records.
- Full UK driving licence and access to a vehicle.
- Willingness to participate in a paid monthly on-call rota.
Desirable
- NVQ/QCF Level 3 or above in Health and Social Care.
- Experience conducting service user assessments and support plan reviews.
- Knowledge of safeguarding and medication management.
