South London and Maudsley NHS Foundation Trust is seeking an experienced and motivated Band 6 Pharmacist to join our Pharmacy team, working across Bethlem Royal Hospital and the Ladywell Unit. This is an exciting opportunity to play a key role in delivering a high-quality clinical pharmacy service across inpatient wards and community mental health teams, supporting medicines optimisation and improving outcomes for service users with complex mental health needs.
As part of a multidisciplinary team, you will provide expert advice on psychotropic medicines, lead on medicines safety and quality improvement initiatives, and support both patients and carers to gain maximum benefit from their treatment. What you can add:
To deliver a comprehensive clinical pharmacy service to inpatient wards as part of the multidisciplinary team, ensuring safe, legal and effective use of medicines in line with Trust policies, procedures and national guidelines.
To undertake medicines reconciliation, discharge counselling and medicines-related consultations with inpatients, including running medication groups and supporting Medicines Management technicians where appropriate.
To support patients to gain maximum benefit from their medicines by ensuring understanding, involving them in treatment decisions, and addressing practical issues that may affect adherence.
To review prescriptions for safety, appropriateness and legality, identifying and resolving medicines-related problems and supporting the reduction of medicines waste.
To support inpatient teams in the safe supply and use of medicines, including discharge planning, accurate labelling, and safe handling of controlled drugs.
To contribute to medicines optimisation and patient safety initiatives, including identification and reporting of medicines-related incidents.
To act as a practice supervisor in the training and development of trainee and junior pharmacists within the clinical setting.
What you’ll need:
NHS hospital pharmacy experience is essential, including experience in a patient-facing clinical pharmacist role.
Mental health experience is desirable, with understanding of psychotropic medicines and monitoring requirements.
A Master’s degree in Pharmacy and current registration with the General Pharmaceutical Council (GPhC), with evidence of ongoing CPD.
Strong knowledge of medicines use, prescribing principles and safe medicines management within secondary care.
