An exciting opportunity has arisen for a band 6 Mental Health Nurse to work within East Dunbartonshire older peoples community mental health service, Care home liaison team You will be an experienced registered mental health nurse who has the ability to work autonomously and also as part of a team. You will be able to demonstrate robust assessment and risk management skills with experience of multi-agency working, partnership working. You will manage a patient caseload, carry out domiciliary visits and provide advice and response to crisis. You will support the Team Lead to deliver effective operational services and service development. You will provide clinical supervision to junior staff.
As an integral member of the Community Mental Health Team & Care Home liaison Team, you will liaise with Care homes, GPs, hospital and primary care teams, social work colleagues to formulate and promote integrated, person-centred care recovery based outcomes.
Excellent communication and interpersonal skills are essential as is knowledge of relevant legislation and evidence based practice. It is essential you are a registered mental health nurse with a current NMC registration with extensive experience at band 5 and evidence of continual professional development. A Working knowledge of evidence based psychological interventions in response to distress in Dementia is essential. Community experience is desirable as within OPMH. This is a permanent post 36 hours and you will work Monday to Friday 9am to 5pm.
The post is East Dunbartonshire-wide initially and you will be expected to work across the care home estate in East and West localities and to work closely with EDHSCPs Care Home Support Team. You will be based at Woodlands OPMH Resource Centre. Owing to the wide geographical catchment area a driving license is essential.

Key responsibilities:
- Functioning as Named Nurse/ keyworker within a multi-disciplinary team for a complex patient caseload requiring specialist mental health care. Undertaking named practitioner responsibilities for patients requiring care coordination where required.

The holistic assessment of complex care needs including risk assessment, planning, implementing and evaluating specialist programmes of care.

Carrying out all relevant forms of nursing care pertinent to the needs of individual patients in the least restrictive and disruptive manner as possible.

Identifying, coordinating and responding to the health promotion needs of patients focusing on improving the mental and physical well-being of patients

Identifying, coordinating and responding to the needs of patients which might be related to vulnerability, gender, health, domestic abuse, adult/ child protection and/ or self harm.

Developing therapeutic relationships with patients and carers with complex/ unstable care needs ensuring that needs are identified and met within a participative framework demonstrating principles of equality, dignity and respect accounting for spiritual and cultural diversity. Working proactively with other agencies/ services (ie interpreter services) to aid effective communication.

Working within agreed competency based parameters undertaking evidence based psychological therapies/ psychological approaches to care with individuals and/ or group settings.
