Job Overview
The Care Co-ordinator at Stella Maris Healthcare Ltd is responsible for overseeing the day-to-day delivery of high-quality, person-centred care within a supported living service. The role involves coordinating staff, ensuring compliance with regulatory standards, maintaining accurate care documentation, and promoting positive outcomes for service users with learning disabilities, autism, and/or mental health needs.
The Care Co-ordinator acts as a key link between service users, support staff, families, healthcare professionals, and local authorities, ensuring care is delivered safely, effectively, and in line with agreed support plans.
Duties – Care Co-ordinator (Supported Living, Birmingham)
- Coordinate and oversee the day-to-day delivery of care to ensure it is safe, effective, and person-centred
- Develop, implement, and regularly review care plans and risk assessments
- Ensure all care documentation is accurate, up to date, and compliant with regulatory standards
- Allocate and manage staff rotas to maintain safe staffing levels at all times
- Provide guidance, supervision, and support to support workers
- Conduct spot checks, audits, and quality assurance checks across the service
- Monitor service user progress and outcomes, making adjustments to support where required
- Promote independence, choice, dignity, and inclusion for all service users
- Manage and report incidents, accidents, and safeguarding concerns in line with policy
- Liaise with families, healthcare professionals, social workers, and commissioners
- Attend and contribute to care reviews, meetings, and multidisciplinary team discussions
- Oversee safe administration and management of medication
- Ensure staff adhere to policies, procedures, and best practice guidelines
- Support staff training, induction, and ongoing development
- Maintain a safe, clean, and supportive living environment
- Participate in on-call duties and respond to emergencies when required
- Contribute to service improvement and ensure readiness for inspections and audits

Requirements
- Level 3 Health & Social Care (essential); Level 5 (desirable or working towards)
- Minimum 2 years’ experience in supported living or similar setting
- Experience with learning disabilities, autism, and/or mental health
- Knowledge of safeguarding, MCA/DoLS, and CQC standards
- Ability to develop care plans and risk assessments
- Strong leadership, communication, and organisational skills
- Competent in record keeping and basic IT systems
- Enhanced DBS and right to work in the UK
- Flexible to work shifts and participate in on-call
