We are excited to launch Sherwood Street Women’s Service, a new 24-hour supported accommodation service designed to provide safe, secure accommodation and specialist support for women experiencing severe multiple disadvanged, domestic abuse, and complex needs.
We are seeking an experienced and passionate Support Planner to take a lead role in supporting residents to achieve positive outcomes, whilst helping shape and develop a brand-new service from the ground up.
This is an opportunity for someone who wants more responsibility than a traditional support worker role and who thrives on building relationships, coordinating services, managing risk, and supporting people through meaningful change.
About the Service
Our service provides accommodation for up to eight women who require a safe environment and tailored support to rebuild their lives and move towards greater independence.
Working as part of a dedicated team, you will play a key role in ensuring residents receive high-quality, person-centred support that is trauma-informed, empowering, and focused on recovery.
What You'll Be Doing
As a Support Planner, you will take responsibility for a caseload of residents and lead on the planning and delivery of their support journey.
Key responsibilities include:

Undertaking comprehensive assessments, support planning, risk management, and regular reviews.
- Building professional, trusting relationships with residents and helping them identify and achieve their goals.
- Leading on complex case management, ensuring support is tailored to individual needs and circumstances.
- Supporting women with issues including homelessness, domestic abuse, mental health, substance use, offending histories, and social exclusion.
- Coordinating support with external agencies including women's services, housing providers, healthcare professionals, probation services, social care, and specialist support organisations.
