The Wound Care Service has an exciting opportunity for a Band 6 Tissue Viability Nurse to join their committed and passionate team. This is a varied role providing expert assessment for patients with complex wounds in a variety of settings and via remote triage.
The service also supports the Integrated Network Teams (INT) in becoming confident in wound care and lower limb management by education, caseload reviews and a supportive in-reach model. The overarching aim is to enable patients to receive best practice care in a timely manner and improve outcomes for them. The role also plays a key part in the prevention of pressure injuries across the organisation.
The service currently runs Monday to Friday 0800-1700. This role is for 30 hour a week.
Work as part of the wound care service providing an efficient and effective service to patients and colleagues within the Sirona care and health community.
Provide specialist tissue viability assessment for patients with complex wounds in a variety of settings and via a rota of remote triage and develop appropriate treatment plans.
Contribute to development and review of evidenced based tissue viability guidelines, protocols and pathways and contribute to embedding these in practice.
Contribute to the pressure ulcer prevention strategy across Sirona in working to embed Sirona protocols in best practice and reduce pressure ulcer incidence. Provide accurate verification of pressure ulcers.
Provide tissue viability education in a variety of ways to clinicians within the Integrated Network Teams (INTs).
