Role Overview
We are seeking a proactive and compassionate Care Co-ordinator to join our team. The ideal candidate will have strong leadership and communication skills, along with experience supervising care staff within a care home or senior care setting. This role involves coordinating care plans, maintaining effective communication with clients and their families, and upholding high standards of service delivery to enhance our clients' well-being.
Job Description
- Broad experience leading teams, creating staff rotas, and delivering work to a high standard, with a flexible approach to the role
- Strong relationship-building skills and the ability to inspire others to achieve team success — this is essential
- Good knowledge of current legislation, with the tact and diplomacy needed to find fair solutions in challenging situations, and the ability to motivate a team and foster a strong working environment

Responsibilities
- Coordinate and schedule appointments for clients and healthcare providers
- Maintain accurate records of client information and appointments
- Communicate effectively with clients, healthcare professionals, and team members
- Assist in resolving client inquiries and concerns in a timely manner
- Collaborate with healthcare providers to ensure seamless care coordination
- Draw on office experience to efficiently manage administrative tasks related to care coordination
Qualifications
- NVQ Level 3 in Health and Social Care
- Office and administrative experience
- Strong organisational skills
- Excellent communication skills
- Ability to multitask and prioritise effectively
- Proficiency in the Microsoft Office suite
- Attention to detail and accuracy in data entry
- Ability to work independently and as part of a team
