Job Summary
The Care Coordinator plays a vital role in ensuring that patients receive comprehensive and coordinated care throughout their healthcare journey. This position involves working closely with healthcare providers, patients, and their families to facilitate communication, manage care plans, and ensure that all necessary services are delivered effectively. Duties
- Collaborate with healthcare professionals to develop and implement individualised care plans for patients.
- Act as a primary point of contact for patients and their families, providing support and guidance throughout the care process.
- Schedule appointments, coordinate referrals, and ensure timely follow-up on patient care.
- Maintain accurate records of patient interactions, care plans, and progress notes in compliance with organisational policies.
- Assist in the management of office operations to ensure a smooth workflow within the healthcare setting.
- Facilitate communication between various departments to enhance patient care and resolve any issues that may arise.
- Monitor patient progress and adjust care plans as necessary to meet changing needs.
Experience
- Previous experience in a healthcare setting is preferred, particularly in roles involving patient coordination or administrative support.
- Strong organisational skills with the ability to manage multiple tasks simultaneously.
- Excellent communication skills, both verbal and written, to effectively interact with patients, families, and healthcare professionals.
- Proficiency in using office software and electronic health record systems is advantageous.
