Are you an experienced Practice Nurse looking to take the next step in your career?
Chartfield Surgery is seeking an exceptional clinician and leader to join our award-winning team as our new Lead Nurse & Clinical Care Transformation Lead.
As we prepare to celebrate the retirement of a highly respected colleague who has dedicated more than 20 years of service to our patients, we are looking for a forward-thinking nurse who can build on that legacy whilst helping shape the future of primary care.
This is far more than a traditional Lead Practice Nurse role.
You will be a key member of our multidisciplinary leadership team, influencing clinical services, leading quality improvement initiatives, developing our nursing workforce and ensuring the highest standards of patient care across the practice.
What You'll Lead
- Inspire and develop a high-performing team of Nurses and Healthcare Assistants
- Drive excellence in long-term condition management and preventative healthcare
- Lead innovation for frailty, care home and housebound patient services
- Shape women's health, men's health and vaccination programmes
- Champion infection prevention and quality improvement
- Support CQC readiness and clinical governance
- Improve patient outcomes through data-driven and evidence-based care
- Help develop the next generation of nursing professionals
About You
We're looking for someone who:
- Is a Registered Nurse with experience in primary care
- Has strong leadership and people development skills
- Is passionate about preventative healthcare and population health management
- Thrives in a collaborative multidisciplinary environment
- Wants to influence and improve services, not simply maintain them
- Is committed to delivering outstanding patient-centred care
Independent prescribing and chronic disease management experience would be highly desirable.
