Medical Receptionist / Care Coordinator
We are seeking a Medical Receptionist / Care Coordinator to join our patient services team across Moor Green Lane Medical Centre and Vicarage Road Surgery.
This is a patient-facing role combining reception duties, telephone triage, patient recall management, and care coordination within a busy GP practice environment.
The role combines:
- Care coordination for patients with long-term conditions and complex needs
- Telephone triage and reception duties
- Management of patient recalls for routine and long-term condition reviews
The successful candidate will help enhance the patient experience by guiding individuals through the healthcare system and ensuring they access the most appropriate services for their needs.
The role focuses on personalised patient support, effective signposting, and collaboration with internal teams and external organisations to ensure smooth care pathways.
Applicants must have previous experience working within a GP practice or NHS primary care environment, including experience using EMIS Web (or a similar clinical system) and involvement in patient recall systems for long-term condition reviews (for example diabetes, asthma or hypertension).
Strong IT skills are essential, as the role includes creating searches, data input, and monitoring progress against national, local and internal targets.
Due to the nature of the role, applications without previous GP practice or NHS primary care experience are unlikely to be shortlisted.
The post holder will be required to work across both Moor Green Lane Medical Centre and Vicarage Road Surgery.
Working hours:
Applicants must be available for 8:00am opening and/or 6:30pm closing shifts on a rota basis.
Key Responsibilities:
Reception Duties & Telephone Triage
- Act as a professional and welcoming first point of contact for patients in person, by telephone, or online.
- Handle telephone triage, assessing patient needs and escalating urgent concerns where appropriate.
- Support appointment bookings, queries, and cancellations using established protocols.
- Promote patient access to digital and online services.
- Register new patients and maintain accurate demographic records.
- Process correspondence and communications.
Recalls Management
- Coordinate patient recalls for long-term condition reviews (e.g. diabetes, asthma, hypertension).
- Work with clinicians and administrative colleagues to plan and schedule recall invitations.
- Follow up with patients to support attendance at required appointments.
- Identify patients overdue for health checks or reviews and arrange appropriate bookings.
- Maintain accurate recall records within EMIS Web and assist with audit or reporting requirements.
Care Coordination & Personalised Support
- Support patients and carers to navigate health, social care, and community services.
- Refer patients to social prescribing link workers, health and wellbeing coaches, or other appropriate services where relevant.
- Assist patients in preparing for or following up on clinical conversations to support informed decision-making.
Digital Support
- Support the use of online consultation systems within the practice.
- Monitor and process digital consultations, ensuring timely escalation and action where needed.
- Assist with improving digital workflows and patient access systems.
Administration & IT Systems
- Maintain accurate patient records using EMIS Web and other systems.
- Assist with searches, data extraction and reporting for QOF, audits and PCN requirements.
- Process incoming and outgoing correspondence, scanning, and task management.
Consultation Room Preparation
- Ensure consulting rooms are prepared and stocked before clinical sessions.
- Clear and re-stock rooms as required.
- Ensure rooms are tidy and secure at the end of sessions.
Person Specification
Applicants must have:
- Previous GP practice or NHS primary care experience
- Experience using EMIS Web or another clinical system
- Experience supporting patient recall systems or long-term condition reviews
- Experience in telephone triage and reception duties
- Strong IT, organisational, and communication skills
- A patient-focused approach and ability to work within a busy team environment
Job Types: Full-time, Part-time, Permanent
Part-time hours: 25-37.5 per week
Pay: £12.71-£14.00 per hour
