Are you an experienced Senior Care Assistant with a minimum NVQ Level 2 in Health & Social Care and a passion for helping people live independently?
Join a welcoming community-based service providing short-term rehabilitation, reablement and end-of-life care for up to 35 clients. You will support people following illness, injury or hospital discharge, helping them rebuild confidence, develop daily living skills and achieve personal goals in a modern, supportive setting.
Working alongside healthcare professionals and therapy teams, you will provide high-quality, person-centred care while offering day-to-day guidance and support to Care and Reablement Assistants.

- Early and late shifts: 7:00am–2:30pm or 3:00pm–10:30pm

Your responsibilities
- Delivering high-quality, outcome-focused reablement and personal care support
- Supporting clients to achieve agreed care plans and independence goals
- Providing practical assistance with daily living activities, mobility, personal care, meals and medication support where required
- Acting as a senior point of contact for Care and Reablement Assistants during shifts
- Supporting, mentoring and guiding junior members of the care team
- Assisting with allocating daily workloads and ensuring visits are completed safely and effectively
- Completing and contributing to initial visits, review visits, care plans and risk assessments
- Communicating professionally with clients, families, healthcare professionals and therapy teams
- Maintaining accurate electronic care records and reporting changes in clients’ needs
- Promoting safe working practices, including the correct use of equipment and PPE
- Ensuring care is delivered in line with policies, CQC Fundamental Standards and the Care Act 2014
What we’re looking for
- NVQ Level 2 in Health & Social Care or equivalent — essential
- Previous experience as a Senior Care Assistant, Care Assistant, Support Worker or Reablement Assistant
- Experience supporting adults in a health, social care, rehabilitation, domiciliary care or reablement environment
- Experience mentoring, supporting or acting as a senior member of a care team
- A full UK driving licence and access to your own vehicle
- Business-use motor insurance
- Strong communication, teamwork and organisational skills
- Confidence working independently and using your initiative
- A caring, reliable and professional approach
- A genuine commitment to delivering person-centred care and promoting independence

This is a community-focused role, requiring flexibility to work early and late shifts across weekdays and weekends. You will support clients with a variety of needs, ensuring they receive the right care and encouragement to maximise independence and quality of life.
