ONGOING ASSESSMENT AND INTERVENTION TEAM - 1 x 37hr Social Worker - fixed term post until 30th September 2027. We are currently looking to recruit an experienced Band 10 or 11 Social Worker.
You will work within a cohesive team of Social Workers, under the supportive guidance of a Team Manager and Senior Practitioner to deliver person-centred outcomes to the people of Darlington. The team are based at the Town Hall in Darlington with a mixture of home and office based working.
You will provide ongoing assessment, support planning and interventions for adults aged 18+ with a range of complex needs in the Darlington area.
The team works proactively with service users, their families, carers and professionals to maximise independence through a comprehensive strengths-based approach to assessment and support planning. The dynamic nature of the team is underpinned by a strong commitment to the principles underpinning The Care Act 2014 and other relevant legislation.
The successful applicant will be required to contribute to the operational delivery of an effective Adult Social Care Service and be responsible for a range of assessments including Care and Support Needs, Mental Capacity Act, Safeguarding, and Support Planning to promote the wellbeing of adults in line with the Care Act 2014.
