Job Summary
We are looking to recruit an experienced and motivated Practice Nurse to join our friendly, patient-focused GP practice in Windsor for four sessions per week.
This is an excellent opportunity for a skilled Practice Nurse with a special interest in diabetes management to become an integral member of our multidisciplinary team. You will provide high-quality, evidence-based nursing care while supporting patients with long-term conditions to achieve the best possible health outcomes.

Main Duties of the Job
The successful candidate will:
· Lead and deliver diabetes clinics, including annual reviews and ongoing monitoring.
· Support patients with Type 2 diabetes, pre-diabetes and cardiovascular risk reduction.
· Develop personalised care plans and provide education to promote self-management.
· Undertake chronic disease management clinics, including hypertension, asthma and COPD (where competent).
· Administer childhood and adult immunisations and vaccinations.
· Perform cervical cytology (if qualified).
· Undertake NHS Health Checks.
· Carry out venepuncture, wound care, ECGs, blood pressure monitoring and other routine nursing procedures.
· Provide health promotion advice including smoking cessation, weight management and healthy lifestyle interventions.
· Maintain accurate clinical records using the practice clinical system.
· Work collaboratively with GPs, pharmacists, healthcare assistants and the wider primary care team.
· Participate in clinical governance, audit and quality improvement activities.
Act as the Infection Prevention and Control (IPC) Lead for the practice, ensuring compliance with infection prevention policies, audits, training and best practice.
Person SpecificationEssential
· Registered Nurse with current NMC registration.
· Minimum of 2 years’ experience working as a Practice Nurse within UK General Practice.
· Proven experience managing diabetes in primary care.
Diploma or accredited qualification in Diabetes Care (essential).
· Competent in chronic disease management.
· Experience in immunisations and vaccinations.
· Venepuncture and wound care skills.
· Excellent communication and organisational skills.
· Ability to work independently and within a multidisciplinary team.
· Commitment to providing compassionate, patient-centred care.
Desirable
· Independent Nurse Prescriber qualification (V300).
· Cervical cytology qualification.
· Experience using EMIS Web.
· Experience delivering QOF and enhanced services.
· Experience initiating or managing injectable diabetes therapies.
