Job Summary
We are seeking a dedicated and compassionate Complex Care Support Worker to join our team, providing high-quality, person-centered care for young people with complex needs. Duties
- Assist young people with daily activities such as personal hygiene, feeding, mobility, and medication administration according to individual care plans
- Develop and implement personalised care plans in collaboration with families, healthcare professionals, and support teams
- Provide emotional support and encouragement to promote confidence and independence in young people.
- Use effective communication skills to liaise with families, healthcare providers, and team members about care needs and progress
- Support young people safely to appointments, activities, or outings ensuring that community risk assessments have been conducted.
- Maintain accurate documentation of care provided, incidents, and updates in IT systems designed for care management
- Monitor the young person's health status closely and respond promptly to any changes or emergencies
Requirements
- Experience working with children/young people in an acute care setting or within a complex care community service.
- Experience as a Healthcare Assistant or Support Worker
- Knowledge of child development principles and ability to adapt support accordingly
- Experience with conducting clinical observations under the leadership of a Registered Practitioner
- Care experience in the field of CAMHS Eating Disorders
- Ability to create and follow detailed care plans tailored to each young person's clinical needs
- Excellent communication skills to build trusting relationships with young people, families, and colleagues
- Proficiency in using IT systems for documentation and reporting purposes
- Strong organisational skills with attention to detail in recording care activities and updates
- In depth knowledge of Safeguarding, Child Protection and the Duty of Candour
