JOB PURPOSE:
The Community Care Coordinators will be responsible for coordinating and monitoring the delivery of high-quality, person-centred care within the community.
The post holders will support clients and care staff through regular field visits, client assessments, spot checks and quality monitoring, while working closely with the Senior Care Coordinator and Registered Manager to ensure safe, effective and responsive service delivery.
KEY DUTIES AND RESPONSIBILITIES:
1. Community Care Delivery: Coordinate and monitor the delivery of care within the community, ensuring care is delivered safely, on time and in accordance with individual care plans. Respond promptly to changes in client needs and escalate concerns where appropriate.
2. Client Assessments and Care Planning: Undertake initial and review assessments, complete risk assessments and contribute to the development and review of person-centred care plans. Ensure care documentation remains accurate and reflects clients' changing needs.
3. Client Reviews: Carry out regular client review visits to monitor satisfaction, evaluate care outcomes and identify opportunities to improve service delivery. Maintain positive relationships with clients and their families.
4. Spot Checks and Competency Assessments: Conduct regular spot checks, observations and competency assessments of care staff to ensure care is delivered safely, professionally and in accordance with company policies and CQC standards.
5. Staff Support: Provide day-to-day guidance and practical support to Healthcare Assistants while working in the community. Mentor new staff, identify development needs and escalate performance concerns to the Senior Care Coordinator or Registered Manager.
6. Care Visit Monitoring: Monitor electronic care records and visit completion to ensure scheduled care visits are completed correctly. Investigate missed, late or incomplete visits and support appropriate corrective action.
7. Medication Support: Monitor medication administration practices during field visits, audit MAR charts where required and report medication concerns promptly. Promote safe medication practices and support medication competency assessments.
8. Safeguarding: Recognise, report and escalate safeguarding concerns in accordance with company procedures and statutory guidance. Maintain accurate safeguarding documentation and contribute to safeguarding investigations where required.
9. Health, Safety and Risk Management: Promote safe working practices by completing environmental and moving and handling risk assessments, monitoring lone working arrangements and reporting accidents, incidents and hazards promptly.
10. Infection Prevention and Control: Promote infection prevention and control standards by monitoring compliance with PPE requirements and safe working practices during community visits. Report concerns and support continuous improvement.
11. Recruitment and Induction Support: Support the induction of new care staff through shadowing, competency observations and practical guidance within the community. Assist with recruitment activities and participate in interviews where required.
12. Training Support: Identify staff training needs during field observations and provide coaching where appropriate. Support the Registered Manager and Senior Care Coordinator in maintaining workforce competency.
13. Communication and Partnership Working: Develop effective relationships with clients, families, healthcare professionals and partner organisations to promote coordinated care and positive client outcomes. Communicate changes in client needs promptly to the office team.
14. Administration and Record Keeping: Maintain accurate client visit records, assessment documentation, spot check reports, competency assessments and other operational records using the electronic care management system.
15. Service Improvement: Contribute to quality assurance and continuous improvement by identifying trends, reporting concerns and making recommendations to improve care delivery and client satisfaction.
16. On-Call Responsibilities: Participate in the out-of-hours on-call rota and provide operational support during emergencies, staffing shortages and urgent client situations.
17. General Duties: Support the Senior Care Coordinator and Registered Manager with operational activities as required and undertake any other reasonable duties consistent with the responsibilities of the role.
REQUIRED SKILLS:
Essential Qualifications
- Level 3 Diploma in Adult Care (or equivalent).
- Care Certificate.
- GCSE (or equivalent) in English and Mathematics.
Essential Experience
- Minimum of 2 years' experience in domiciliary or home care.
- Experience delivering person-centred care within the community.
- Experience undertaking client assessments and risk assessments.
- Experience conducting spot checks and competency observations.
- Experience using electronic care management systems.
- Experience supporting and mentoring care staff.

Pay: £36,000.00-£38,000.00 per year
