AMLINELLIAD
I gael cyfrifoldeb yn unol ag ymarfer a dull gweithredu Age Cymru Gwynedd a Môn ar gyfer anghenion personol a domestig unigolion fel y nodir ar y cynllun gwasanaeth. Nôd y gwasanaeth yw sicrhau parhad annibyniaeth yr unigolyn yn y gymuned gan ddarparu cymorth a chefnogaeth addas i’r unigolyn a’r gofalwyr anffurfiol. Bydd asesiad cychwynnol o anghenion yr unigolyn yn cael ei ymgymryd gan y Rheolwr Gofal Cartref/Cymunedol neu’r Goruchwyliwr a bydd Cynllun Gwasanaeth yn cael ei gwblhau. AGE CYMRU GWYNEDD A MON DOMICILIARY CARE SERVICE
JOB DESCRIPTION
POST: Community Support Worker (Felinheli Area) Salary: £12.82 per hour
RESPONSIBLE TO: Community/Home Care Manager
GENERAL INFORMATION
Age Cymru Gwynedd a Môn is committed to providing social care and support services of the highest standard and quality to promote the independence, dignity and well-being of individuals to enhance their quality of life enabling them to remain in their own homes.
OUTLINE
To have responsibility in accordance with the practices and procedures of Age Cymru Gwynedd a Môn for the personal and domestic needs of individuals as specified on the service plan. The service aims to ensure the continued independence of the individual in the community by providing appropriate help and support to both individual and his/her informal carers. The initial assessment of the individual’s needs will be undertaken by the Domiciliary Care Manager or Supervisor and a Service Plan outlining the care needs will be drawn up, taking into account the carer’s individual availability relating to hours of work, this will normally be agreed between the Domiciliary Care Manager/Supervisor and the carer. The post involves evenings, weekend and bank holiday working.
MAIN DUTIES AND RESPONSIBILITIES
General personal care duties which could include assisting individuals in and out of bed, assisting with dressing/undressing, washing, feeding, bathing,
Social duties to include forming a relationship with the individual by listening and talking with the individual and carer and assisting the individual’s contact with family, friends and the community
Report any concerns/changes regarding any individual to the Manager
Participate and attend staff meeting, supervision, appraisal and training as required by the Manager.
To maintain accurate, concise and up to date records of the individual’s care by recording in the log books provided.
To ensure any documentation such as timesheets and travel expenses are correct and on time.
Confidentiality is to be maintained and respected at all times.
Notify the Manager immediately when unable to carry out services.
The carer may be required from time to time to undertake other duties that commensurate with the post not specifically mentioned in the job description.
PERSON SPECIFICATION

The minimum educational /training requirements for the post:

QCF Level 2 / relevant experience
First Aid
Food Hygiene

Specific Training required once in post

3.1 Essential
- The post holder must hold a full current driving licence with the appropriate business user insurance cover and must have access to a car. Travel expenses will be paid.
- An acceptable enhanced DBS check.
