Senior Primary Care Nurse
Location: South Gloucestershire
Salary: £43,609 - £49,175 per annum (pro rata, depending on experience)
Contract Type: Permanent
Hours: Part-time, 25 hours per week
Shift Pattern: Long days and catch-up shifts (07:00 – 19:30 / 07:00 – 10:30)

Sanctuary Personnel are recruiting for an experienced Senior Primary Care Nurse to join a specialist healthcare team within a prison setting near Bristol. This is a rewarding opportunity for a Registered Nurse to provide high-quality, evidence-based care while leading and supporting a multidisciplinary team within a secure healthcare environment.
The successful candidate will play a key role in delivering nurse-led primary care services, improving patient outcomes, and ensuring individuals receive safe, compassionate, and personalised healthcare.
Key Responsibilities
- Lead and support a multidisciplinary healthcare team to deliver effective nurse-led primary care services.
- Provide high-quality, evidence-based nursing care to individuals within a secure environment.
- Complete patient assessments, reception screening, and clinical reviews.
- Undertake medication rounds and administer controlled medications safely and appropriately.
- Respond to emergency situations and provide clinical support when required.
- Work proactively to reduce avoidable hospital admissions through effective care planning and intervention.
- Deliver compassionate, non-judgemental care that focuses on individual patient needs.
- Promote clinical excellence through supervision, mentoring, and sharing best practice.
- Support audits, governance processes, quality improvement initiatives, and evidence-based practice.
- Build strong working relationships with internal teams and external healthcare partners.
- Encourage a culture of learning, collaboration, and continuous improvement.
Requirements
- Registered Nurse (Adult) with a current NMC registration.
- Previous experience working as a Registered Nurse within primary care, community, hospital, or secure healthcare settings.
- Experience providing clinical leadership and supporting multidisciplinary teams.
- Experience with line management, clinical audits, and healthcare governance.
- Evidence of Continued Professional Development (CPD).
- Strong understanding of multidisciplinary and multi-agency working.
- Excellent communication, leadership, and interpersonal skills.
- Ability to provide safe, compassionate, and patient-centred care in a challenging environment.
- Mentorship/assessor qualification and non-medical prescribing qualification are desirable.
