We are looking for an ambitious and dedicated Social Worker to play a pivotal role within the interdisciplinary team at the NRC. This role is integral to supporting patients and their families throughout their rehabilitation journey, coordinating patient pathways, supporting timely discharge planning, and facilitating access to community resources and services.
This is an exciting opportunity to join a brand-new purpose-built rehabilitation environment, shape the development of an innovative rehabilitation service, and contribute to the delivery of rehabilitation excellence. The post holder will be a key advocate for patients and families, helping to navigate challenges and enabling individuals to maximise their independence and quality of life.
The post holder will work closely with patients, carers, and professional colleagues to address complex social, emotional, and practical needs, ensuring person-centred care remains at the heart of the rehabilitation process.
