An exciting opportunity has arisen for a part time Occupational Therapist within our Healthy Homes team.
The post holder will hold the relevant qualification in Occupational Therapy and up to date HCPC registration. The post holder will also possess strong clinical reasoning and a client centred approach. The role entails conducting a holistic Occupational Therapy assessment in the homes of older and disabled people. Responding to direct referrals from GP practices, the role facilitates a preventative service approach to enable people to continue living safely and independently in their own homes:
- Assess clients’ physical, cognitive, and emotional needs using recognised assessment tools.
- Develop and deliver personalised plans that promote independence and participation in daily life.
- Recommend and source aids and equipment or environmental adaptations to support client goals.
- Work collaboratively with families, carers, and multidisciplinary teams to ensure holistic care.
- Keep accurate, timely records.
You will also work closely with our Caseworkers and colleagues across health, housing and the social care sector.
Full driving licence and access to a car is essential and subject to an enhanced DBS check.
15 hours per week
Salary £39,562 (pro-rata)
Fixed term: 8 months
