Job Summary
The Ethnic Minority Hospital Advocate & Discharge Worker (EMHADS) plays a key role in improving health outcomes and reducing health inequalities for ethnic minority communities across Wirral. Working in partnership with NHS hospitals, community organisations and other agencies, you will provide advocacy, practical support and guidance to patients and their families throughout their hospital journey.
You will support patients in understanding and accessing healthcare services, assist with discharge planning, help overcome language and cultural barriers, and ensure individuals are connected to the appropriate community services following discharge. The role requires excellent communication, empathy and organisational skills, with a strong commitment to equality, inclusion and person-centred support.
Duties
- Provide advocacy and practical support to patients and their families throughout their hospital journey.
- Support safe and effective hospital discharge planning.
- Liaise with NHS staff, interpreters and partner organisations to ensure coordinated care.
- Carry out client assessments and develop appropriate support plans.
- Signpost and refer clients to health, wellbeing and community support services.
- Help patients overcome language, cultural and social barriers to accessing healthcare.
- Maintain accurate records and case notes in accordance with GDPR and organisational policies.
- Contribute to monitoring, reporting and achieving project outcomes.
- Promote the EMHADS service through community engagement and partnership working.
- Attend meetings, training and outreach events as required.
Requirements
- Fluent in Bengali (essential).
- Excellent communication and interpersonal skills.
- Experience in health, social care, community support, advocacy or a related field.
- Ability to work independently and as part of a multidisciplinary team.
- Good IT and administrative skills.
- Strong organisational skills with the ability to manage a varied workload.
- Understanding of equality, diversity and the challenges faced by ethnic minority communities.
- Knowledge of safeguarding and confidentiality principles.
- Ability to speak Urdu, Hindi or another community language is desirable.

- Birkenhead CH41 4AF: reliably commute or plan to relocate before starting work (required)

- This role is primarily based at Arrowe Park Hospital but also involves travelling to interpreting and translation assignments, other healthcare settings and community locations across Wirral when required. Are you able to travel reliably between these locations?
