We can provide a wide range of clinical care in the community & need a special children’s healthcare support worker to help us do this.
Our caseload can include a range of clinical care needs which will develop & maintain your clinical skills e.g. growth monitoring, blood pressure monitoring, baseline observations, wound care, enteral tube routine care, oxygen monitoring, venepuncture and some injectable medicines.
We will ensure you are supported by a robust competency based training & all our children’s care is led by the supportive Children’s Community Nurses within our team.
You could be working on your own in a clinic, supporting a colleague in a patients home/school visit or needed to support in our office base. Every day is different and making a difference to the lives of babies, children and young people is very rewarding.

The post holder will work as part of the Children’s Community Nursing Team, in the delivery of clinical care to children and young people who have additional health needs.
The post holder will act under the direction of the Children’s Community Nurse (CCN-Registered Nurse) to deliver specific delegated nursing interventions to children and young people who have additional health needs and their families. This may also include children who are nearing the end of their life.
They will work in a range of community settings appropriate to the child and /or young person e.g. schools, clinics or homes to support the smooth running of the CCN service.
To assist the Children’s Community Nurses to ensure samples arrive at the laboratory in a timely manner and clinical supplies and equipment are safely maintained and available for direct patient care when needed.

Main Responsibilities
1. To provide delegated clinical care to children and young people with additional health needs, for which, training and competency assessment will be required, including: ·
clinical observations
urinalysis
blood sampling
wound/stoma management
care of enteral feeding tubes
2. Records patient observations and changes to patient clinical conditions accurately on electronic patient record.
3. To be aware of the expected observation values for the individual child.
4. Identifies and reports changes in a child or young person’s condition, in a timely manner to parents, CCN or emergency services and implements, individual escalation treatment plans, where trained to do so.
5. To maintain high standards of cleanliness and stock management in clinical environment and actively promote the control of infection.
6. To contribute to health promotion activities relevant for the caseload
e.g. Toilet training, healthy eating, continence, infection control,
7. To work together with patients, their parents and the multi-disciplinary team promoting self-care and reducing medicalisation of their daily activities.
8. Acts as chaperone for vulnerable children and young people as per Trust policy.
9. To transport any urgent samples to the hospital lab.
10. Completes safety checks of clinical devices and clinic equipment and reports any unsafe environment/ use of equipment to the Lead Children’s Community Nurse.
Communication
To participate in ensuring effective communications are maintained within the team by attending daily huddles, team meetings, supervision and 1 to 1 meetings with their line manager.
To communicate with babies, children and young people, their families /carers using a range of verbal and non-verbal communication techniques and tools.
To complete accurate patient records in line with the Trust policy, using electronic Systemone records and patient held records, where appropriate.
4. To liaise with the family and multi-disciplinary team when delivering the child’s care recognising individual expressed beliefs, preferences and choice.
5. Promptly alerts the relevant person when there are unexpected changes in individuals’ health and wellbeing or risks.
Actively contributes to the inter-agency team working approach to advocate that the child’s health, social and educational needs are supported at all times.
Financial Responsibility
1. Supports the Team manager to organise and maintain resources and equipment to be available for clinical care in line with appropriate policies and procedures e.g. infection control, medical devices, stock ordering.
Professional and service development
1. To undertake relevant training to ensure own clinical practice is of a safe standard in accordance with Trust policies.
2. To support visiting trainees, students and colleagues when learning clinical skills that the post holder has been assessed as competent in.
3. Maintaining own professional development and requirement to take part in appraisal and personal development process
Clinical and Practice Governance
1. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.
2. Any data that is taken/shared as part of a phone call or transported, faxed, or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
3. The post holder must adhere to the Trust risk assessment and risk management processes
4. The post holder must adhere to infection control policies and procedures
5. Undertake mandatory training and any other training relevant to the role as required by the Trust.
6. The post holder must participate in clinical and safeguarding audits as required
7. Provide patients and their families /carers with information on standards they should expect from the team
8. Participate in Clinical & Safeguarding supervision on a regular basis
9. The post holder is required to participate in relevant emergency preparedness process for their team.
Research & Development
May be required to undertake surveys or audits, as necessary to own work; may occasionally participate in R&D, clinical trials or equipment testing.
Infection Control
The trust staff are responsible for protecting themselves and others against infection risks.
All staff regardless of whether clinical or not are expected to comply with current infection control policies and procedures and to report any problems about this to their managers.
All staff undertaking patient care activities must attend infection control training and updates as required by the trust.
All clinical staff or any staff working in clinical areas are expected to be bare below the elbow with no hand or wrist jewellery except for one plain band (no stones or engraving)
Health and Safety
Employees must be aware of the responsibilities placed upon them under the Health and Safety at Work Act (1974), to ensure the agreed safety procedures are carried out to maintain a safe environment for employees and visitors.

Sustainability

Where the role includes the ordering and use of supplies or equipment the post holder will consider the environmental impact of purchases.
