Who we are
We have supported adults with learning disabilities, autism, and mental health conditions to live independently in their own homes for over a decade. We have no external shareholders. The people we support, and the people doing the supporting, are who we answer to.
We are recruiting for three transitions support workers, one for each of our localities:
East Dorset - areas include: Bournemouth, Poole, Wool, Wareham, Christchurch
West Dorset - areas include: Weymouth and Dorchester
Wiltshire - areas include: Salisbury, Westbury
As a Transitions Support Worker, you will play a key role in supporting people through person-centred transitions to live independently, ensuring their needs, aspirations, and outcomes remain at the heart of the process.
We are looking for individuals who are calm, confident, open-minded, and solution focused, with excellent communication skills.
Experience around risk assessing and support planning is preferred.
Previous experience with autism, learning disabilities and mental health support is essential.
Due to the nature of this role, we require candidates with a full UK driving licence and access to your own vehicle.
