Prescribing Pharmacist – GP Surgery
Location: Basingstoke, Hampshire
Salary: Up to £60,000 per annum, depending on qualifications and experience
Hours: Full-Time/Part-Time (full-time hours can be worked over 4 days)
About the role
We are looking for a motivated and patient-focused Prescribing Pharmacist to join our GP surgery team.
Working alongside GPs, nurses and the wider multidisciplinary team, you will play a key role in delivering safe, effective and patient-centred care.
Key responsibilities
- Conduct telephone triage and clinical consultations, including face-to-face reviews where appropriate.
- Review and action pathology results.
- Undertake medication and clinical reviews for patients with long-term conditions and complex health needs.
- Review hospital correspondence and manage medicines reconciliation and treatment changes.
- Lead the repeat prescription service and respond to prescription queries.
- Liaise with community pharmacies and other healthcare professionals.
- Support medicines optimisation, safe and cost-effective prescribing, and practice clinical initiatives.
- Contribute to improving patient outcomes and experience.
About you
We are looking for a pharmacist who is:
- GPhC registered.
- Clinically focused with excellent communication and consultation skills.
- Patient-centred, proactive and committed to improving care.
- Able to work effectively within a multidisciplinary team.
- Keen to develop their clinical skills and career in primary care.
Previous primary care experience is desirable but not essential.
