What You'll Be Doing
As our Care Coordinator, you will play a key role in ensuring our care services run smoothly every day. Your responsibilities will include:
- Coordinating and maintaining care staff rotas
- Scheduling care visits efficiently while ensuring continuity of care
- Supporting the Registered Manager with the day-to-day running of the service
- Liaising with clients, families, healthcare professionals and care staff
- Responding promptly to changes in care needs and staffing requirements
- Completing administrative tasks and maintaining accurate records
- Assisting with the onboarding and support of new care staff
- Conducting quality assurance (QA) visits to ensure the highest standards of care are consistently delivered
- Completing service user reviews and working with individuals and their families to ensure care plans remain accurate and person-centred
- Carrying out initial care assessments and reassessments, developing and updating care plans in line with individual needs and company procedures
- Monitoring service quality and identifying opportunities for continuous improvement
- Ensuring services remain compliant with CQC standards, company policies and regulatory requirements
- Participating in the on-call rota where required
- Travelling occasionally to our services in Harrogate and Beverley
What We're Looking For
Essential:
- Minimum 2–3 years' experience in a senior support worker or care coordination role within domiciliary care.
- Good understanding of CQC regulations and person-centred care
- Excellent organisational and communication skills
- Ability to manage multiple priorities in a busy environment
- Strong IT skills and confidence using care management systems
- Full UK driving licence and access to your own vehicle
- Flexible, reliable and professional approach
- Female applicant (Genuine Occupational Requirement applies)
Desirable:
- Experience working closely with a Registered Manager
- Knowledge of rostering software
- NVQ Level 3 in Health & Social Care (or willingness to work towards)
