This post is offered as a fixed term contract until 31 March 2027, or as a secondment for internal staff.
The Senior Case Worker is part of the Hammersmith and Fulham Navigator Service, made up of three Senior Case Workers, a Co-ordinator and a Service Manager.
The Navigator Service provide intensive support to vulnerable adults who are:

At risk of homelessness, e.g. due to eviction or difficulties sustaining their tenancy,

In Supported Housing and need intensive support to be able to move on into their own independent tenancy

Homeless (e.g. sleeping on the streets / sofa surfing / leaving hospital or prison with no accommodation) and are needing intensive support to find suitable accommodation
About you
The Senior Case Worker will carry their own caseload of clients who they will be responsible for delivering intensive case work to. Navigator clients may have multiple and complex support needs, including substance misuse, mental health, physical health, and offending behaviours.
The Navigators work to empower clients in their recovery from homelessness and/or on their journey to independence.
To achieve this, we:

Are flexible and client-centred. We breakaway from a one-size-fits all approach, instead we provide dynamic and flexibility support that offers clients choice in how we work with them

Develop expert skills and knowledge to support clients to navigate their way through services, e.g. housing, benefits, health, substance misuse etc.

Use personalised budgets to ensure clients have the resources they need to live safe, happy lives and progress on their recovery journey

Work in a trauma informed manner, taking into account clients psychological and emotional needs.
